GENERAL RELEASE AND CONSENT
In consideration of its agreement to consider using my Material in its marketing and publicity materials, I, the undersigned, hereby give my irrevocable release, permission, and consent to Franklin Covey Co. (“FranklinCovey”), its agents, successors, subsidiaries, and assigns to use, print, publish, display, communicate to the public, rent, lend, reproduce, alter, modify, adapt, create derivative works of, and sublicense, and use all my rights under copyright, performers’ rights and other intellectual property, in any manner whatsoever throughout the world and in perpetuity, my writing, quote, interview, personal story, testimonial, endorsement, photograph, audio and/or video content, illustration, and/or other material I provide to FranklinCovey, including my name, image, voice and likeness (collectively, “Material”), for any purpose connected to FranklinCovey’s business. I understand FranklinCovey may modify, abridge, alter, adapt, and otherwise create derivate works of the Material, and I consent to FranklinCovey engaging in any activity or omission that would, but for this consent, infringe any moral rights or other non-assignable personal rights I may have in the Material. I further acknowledge that FranklinCovey may use and disclose the Material in connection with FranklinCovey’s marketing efforts, including posting the Material online and using the Material in connection with email marketing. I acknowledge that FranklinCovey and its agents, successors, subsidiaries, and assigns will collect, use, process, store, adapt, alter, transfer, disseminate and disclose personal information in the Material and that I provide when submitting the Material as set out in its Privacy Policy available at: https://www.franklincovey.com/about/privacy-policy/.
I confirm that I am over the age of 18.
I grant the permission contained herein with the understanding I will not receive any compensation or royalties, now or in the future, for the use of the Material by FranklinCovey.
This general release and consent will be governed by the laws of the State of Utah in the United States of America.
